Establish the exposure state
Information may be accessible to an application without being retrieved. Retrieved information may be processed without being displayed or delivered outside its intended boundary. The investigation needs evidence for each transition.
Trace the data owner and recipient
Identify source permissions and the identities involved. Review application, identity and destination records where available. Use the minimum sensitive material needed to establish facts and preserve it under agreed access controls.
Coordinate decisions with the right owners
The incident lead, data owner, privacy team and legal advisers need a consistent factual account. State timestamps, affected populations, known recipients and limitations. Do not substitute an AI-generated narrative for evidence.
Fix the underlying boundary
If broad sharing, connector permissions or an application identity enabled the event, address that control. Review the AI workflow as part of the same chain and validate recovery against the defined data-access requirement.
Write the impact statement in layers
| Evidence state | Report wording should establish |
|---|---|
| Permission only | The data was accessible to the identified principal |
| Retrieval confirmed | The specified material entered the recorded context |
| Output confirmed | The recorded output contained the specified information |
| Delivery confirmed | An identified recipient or destination received it |
| Evidence missing | The unresolved question and record needed to answer it |
Use our AI data leak response to turn these questions into an agreed scope.
